Community Care Live in Wales is seeking an AMHP to balance office-based teamwork with remote working, three days in the office and two days at home, supporting flexibility for personal or childcare needs.

You will assess, plan and coordinate services for individuals, working with other health professionals and agencies to ensure safe, effective care.
