Adults Social Worker needed for Learning Disability Team in East Sussex, paying up to £38ph!

About the Role:

We are seeking an experienced Adults Social Worker to join our Learning Disability Assessment Team, providing high‑quality statutory social work to adults with learning disabilities and their families. This is a rewarding opportunity to make a real difference by promoting independence, improving outcomes, and ensuring that adults with learning disabilities are supported to live fulfilling, safe, and meaningful lives within their communities. You will work within a multidisciplinary environment alongside health professionals, commissioners, providers, and community services to deliver person‑centred, strengths‑based assessments and care planning.

Key Responsibilities:

  • Undertake Care Act 2014 assessments, reviews, and support planning
  • Work with adults with learning disabilities, their families, and carers to promote independence and wellbeing
  • Carry out strengths‑based, person‑centred assessments and outcome‑focused interventions
  • Develop, implement, and review care and support plans
  • Work collaboratively within a multi‑agency health and social care environment
  • Identify and respond to safeguarding concerns in line with statutory responsibilities
  • Apply relevant legislation, policies, and best practice guidance to decision‑making
  • Maintain accurate and timely case recording and documentation
  • Contribute to Mental Capacity Act (MCA) and Best Interests processes where required
  • Support transitions planning where appropriate (including from children’s to adults’ services)
  • Participate in duty rotas and respond to urgent referrals as required
