We are currently seeking an experienced Locum Senior Social Worker to join a Hospital Team. Do not apply unless you hold a recognised Social Work qualification and a minimum of 2 years post qualified experience.
About the Role
This is an excellent opportunity for an experienced Locum Social Worker to join the Hyper-Acute and Acute unit. Working across a Youth Ward and a Community Neuro-Rehabilitation Team, the position plays a key role in supporting complex stroke patients through discharge and rehabilitation pathways.
Working within a specialist multidisciplinary team including Occupational Therapists, Physiotherapists, Speech and Language Therapists, Neuropsychologists, Clinical Nurse Specialists, and Therapy Assistants, contribute to high-quality patient care and safe discharge planning. Duties will include attending safeguarding and family meetings, supporting discharge planning, and providing specialist social care guidance to both patients and professionals.
About You
The ideal candidate will hold a recognised Social Work qualification (Degree/DipSW/CQSW) and have strong experience within hospital-based social work, complex discharge planning, and multidisciplinary team working. Confidence working within fast-paced acute settings and supporting patients and families through challenging transitions is key to the success of this role. Knowledge of The Care Act, safeguarding, housing, benefits advice, and community support pathways is essential.
