About The Role;
Carry out Care Act assessments and reviews within the community Support adults with identifying needs, strengths, and appropriate support plans Work collaboratively with service users, families, carers, and partner agencies Promote independence and person-centred practice Maintain accurate and timely case records and assessments Travel across rural areas within Lewes and North Wealden to undertake community visits Due to the nature of the role, the successful candidate must be a car user.
Key Responsibilities:
To be considered for this position, you will:
Be a qualified and registered Social Worker Have at least 2 years’ recent experience working within a similar Adult Social Care or community-based team Have experience completing Care Act assessments and reviews Be confident managing community-based caseloads independently Possess strong communication and assessment skills Be organized, adaptable, and committed to delivering high-quality support
