Adult Social Worker - Prevention Team - Lancashire Council - £33 -35 per hour
Prevention Service
Location - Fylde and Wyre- Thornton office

The role
In this Social Work role you will complete Care Act assessments and participate in Multi-Disciplinary meetings to consider eligibility for Continuing Health Care for individuals who have been discharged from hospital with Discharge Support funding. These individuals will typically be in a bed based setting.
You will work to defined timescales and promote interventions based upon the client's strengths and focus on what is important to them, offering a truly holistic strength based approach. You will have the ability to engage with service users, their carers and system partners to undertake these assessments virtually and to commission appropriate care to promote independence and meet identified needs.
The post on offer is a permanent, 37 hour contract working Monday to Friday.
What we're looking for
We are looking for an enthusiastic Social Workers with a passion for promoting independence and supporting the Discharge to Assess model.
You will need to be flexible and adaptable in a constantly changing environment as well as having a commitment to improve the lives of our service users through creative, evidence based practice.
Social Work applicants must be registered with Social Work England (SWE) to be appointed as a Social Worker or have applied to register with SWE.
