Are you passionate about empowering adults with learning disabilities to live fulfilling, independent lives? Havering is looking for a dedicated Social Worker to join our Community Learning Disability Team, where you'll play a vital role in delivering personalized, strengths-based support that helps people achieve the outcomes that matter most to them.
Working as part of an integrated multi-disciplinary team, you'll collaborate with health professionals, community partners, service users, and carers to provide high-quality, person-centred social work practice.
Key Responsibilities:- Manage a varied caseload, including complex cases where appropriate.
- Complete person-centred assessments, reviews, and care planning.
- Undertake safeguarding enquiries in line with statutory responsibilities.
- Support safe hospital discharges and help prevent unnecessary admissions.
- Develop and review personalized support plans that promote independence and wellbeing.
- Work collaboratively within a multi-disciplinary team and alongside partner agencies.
- Maintain accurate, timely, and high-quality case records.
- Contribute to service development and, where appropriate, provide supervision and support to colleagues.
- Your own dedicated specialist consultant.
