Community Care Live in Manchester is seeking an experienced Advance Practitioner Adult Social Worker to join a Community Mental Health Team, delivering high-quality support to adults with mental health needs. You will assess and develop care plans, undertake Mental Capacity Act and Care Act assessments, and conduct Section 42 safeguarding enquiries, with hybrid working up to three days home per week.
