Community Care Cooperative (C3) is seeking an experienced Community Health Worker for a hybrid, part-time float pool. The role supports members with complex needs across care teams, with outreach, scheduling, and coordination of services in a flexible, remote-based setting.
The CHW will engage in community outreach, build relationships, assist with Medicaid processes, and support care plans. A valid driver’s license and vehicle are required; experience with EHRs and MS Office is essential.
