We're looking for an organised, practical and people-focused Hospital Discharge Grants Coordinator to help older and disabled people return home safely from hospital. You'll work directly with patients, assess their needs, and coordinate grant-funded repairs, equipment, deep cleans and other essential work, liaising with contractors, hospital teams and community colleagues. This is a varied role offering autonomy and the opportunity to make a genuine difference to people's lives.
Possability People is a pan-impairment disability charity based in Brighton. We work with people with a number of health conditions, including mental health, as well as older people.
Our Hospital Discharge Grants Service helps older and disabled people return home from hospital by arranging grant-funded repairs, adaptations, equipment and other essential work to make their homes safe and welcoming.
We're looking for an organised, practical and people-focused Hospital Discharge Grants Coordinator to join our team.
**About the role**
This is a varied, hands-on role combining administration with visits to people in hospital and at home. You'll work directly with patients to understand their needs, assess potential risks and identify what needs to be done to make their homes safe for their return.
You'll take responsibility for coordinating grant-funded work, liaising with patients, hospital and community professionals, contractors and other organisations. This might involve arranging a deep clean, replacing essential household appliances, organising urgent repairs or making a home safe for someone's return.
You'll also help maintain accurate records, track grant expenditure, monitor service outcomes and contribute to quarterly monitoring and year-end evaluation reports.
You'll need to be comfortable working independently, managing competing priorities and adapting when plans change, while working closely with the rest of the team.
**What you might love about this role**
No two days are quite the same. You'll combine visits to people in hospital and at home with coordinating contractors, organising essential work and building relationships with hospital and community teams. You'll have the autonomy to manage your workload and see things through, while working closely with a supportive team.
Most importantly, you'll make a genuine, practical difference to people's lives by helping them return safely to the place where they're happiest and most comfortable.
**Working arrangements**
The role is based at our head office in Kemp Town, Brighton, with visits to Royal Sussex County Hospital, Brighton General Hospital and clients' homes across Brighton and Hove. Flexible and home working may be possible, and occasional work outside normal hours may be required.
There are no direct reports. You'll work closely with another full-time Grants Coordinator and a part-time manager.
**Salary and benefits**
Salary: Β£28,850 per annum (full-time, 35 hours per week).
Annual leave: 28 days plus bank holidays, pro rata for part-time staff.
You don't need previous experience in hospital discharge. We're interested in transferable skills and experience gained through paid work, volunteering or other relevant activities.
