The predicted salary is between 22000 - 29000 £ per year.
Hours: 37 hours per week Monday to Friday. To include occasional weekend and evening work for which time off in lieu will be arranged.
Salary: £25,750 per annum

How about joining the team at Community Action Derby as one of our Community Link Workers for Derby City South Primary Care Network (PCN)! The Derby City South PCN is a collaboration of 6 GP practices in Derby & South Derbyshire and is committed to improving the care of patients in the city, working with Community Action Derby to take forward Social Prescribing in the area.
Social Prescribing empowers people to take control of their health and wellbeing through referral to ‘Link Workers’ who give time, focus on ‘what matters to me’ and take a holistic approach to an individual’s health and wellbeing, connecting people to community groups and statutory services for practical and emotional support.
If you are a self-motivated, dedicated ‘people-person’ whose passion is to support and empower individuals to take control of their health and wellbeing, you could be just what we are looking for. If you are a creative, innovative thinker who would love the challenge of working across exciting new partnerships within the Primary Care Network and working collaboratively with NHS and Voluntary Sector colleagues in Derby, this could be the job for you.
You will adopt a holistic, non-medical, person-centred approach to your caseload, co-designing action plans to improve health and wellbeing based on ‘what matters’ to the person and their individual strengths and needs – and supporting them to achieve their goals.
Ideally, you will be able to speak a community language and have worked in a similar role before, but this position would also be suited to someone with good local knowledge, appropriate experience and enthusiasm. If you are proactive, enthusiastic and person-centred, then we would love to hear from you.
