Join Comfort Keepers as a Disabilities Case Manager in Enfield, Co. Meath. This role involves working closely with clients and community-based care teams to deliver high-quality, person-centred support in a home setting. It is ideally suited to someone who excels in leadership, care coordination, and client advocacy.
Key Responsibilities- Oversee and coordinate individualised care packages, ensuring safe, effective, and tailored support
- Provide clinical leadership, supervision, and guidance to community-based staff
- Develop, implement, and regularly review personalised care plans
- Liaise with families, multidisciplinary teams, and healthcare professionals to ensure integrated service delivery
- Conduct audits, compliance checks, and staff training to maintain regulatory and quality standards
- Support recruitment, onboarding, and supervision of care staff
- Assist with rostering and scheduling to ensure smooth operational delivery
- BSc in General Nursing or Registered Intellectual Disability Nursing (experience in ABI is desirable)
- 2–5 years’ experience in community or general nursing
- Active NMBI registration (essential)
- Full driver’s licence and access to a private vehicle (travel required)
- Strong IT proficiency, including Microsoft Word and Excel
- Excellent organisational skills with a proactive and structured approach
- Strong communication and leadership abilities
