Referral & Assessment Coordinator Immediately Available Opportunity Comfort Care Services | Slough HQ, Berkshire | Permanent | Full-time | Monday to Friday, 9.00am to 5.00pm About the opportunity We at Comfort Care Services are looking for an experienced Senior Administrator to join our Assessments Team as a Referral & Assessment Coordinator. You will take ownership of the administration supporting each referral from initial enquiry through assessment, funding and move-in. This role will suit someone who works confidently and independently, uses initiative and can keep complex information, deadlines and stakeholders moving without close supervision. Experience within health care, social care, mental health or supported living is highly desirable. Key responsibilities * Act as the first point of contact for new referrals, ensuring information is recorded accurately and progressed promptly. * Coordinate assessments, calendars, documentation and communication with assessors and relevant stakeholders. * Prepare and update support plans, risk assessments, compatibility checks, nomination forms and move-in information. * Maintain accurate vacancy, capacity, service user, funding and hours information across databases, spreadsheets and internal systems. * Follow up referrals, costings and funding decisions, keeping clear records and providing timely updates. * Liaise professionally with assessment, operations, care, finance, housing providers and external partners. * Maintain complete, confidential and audit-ready service user records. * Produce regular vacancy, capacity, hours and portal updates to agreed schedules. What we are looking for * Senior administration experience: Experience working within a demanding but friendly office and team. * Independent working: able to prioritise, make sound day-to-day decisions and progress work with minimal supervision. * Self-motivation: proactive, organised and confident following up actions and keeping work moving. * Excellent IT and systems experience: confident using databases, spreadsheets, strong working knowledge of Microsoft Excel, Word, PowerPoint, Outlook and Teams * Excellent communication skills: clear and professional written and verbal communication with internal and external stakeholders. * Accuracy and judgement: exceptional attention to detail, discretion and professionalism when handling sensitive information. * Sector experience: health care, social care, mental health or supported living experience is highly desirable. * Education: A-level English and Maths, or equivalent, or above.
