Care Coordinators play an important role within our practice to proactively identify and work with people, including the frail/elderly and those with long-term conditions, to provide coordination and navigation of care and support across health and care services. They work closely with GPs and practice teams to manage a caseload of patients, acting as a central point of contact to ensure appropriate support is made available to people and their carers; supporting them to understand and manage their condition and ensuring their changing needs are addressed.

This is achieved by bringing together all the information about a person's identified care and support needs and exploring options to meet these within a single personalised care and support plan, based on what matters to the person. Care coordinators could provide time, capacity and expertise to support people in preparing for, or following-up, clinical conversations, enabling them to be more actively involved in managing their care and supporting them to make choices that are right for them. Care Coordinators help people improve their quality of life. The role is 30 hours a week over 5 days.

Main duties of the job:

  • Provide coordination and navigation for people and their carers across health and care services, working closely with health care professionals to provide collaborative care.
  • Raise awareness of how to identify patients who may benefit from shared decision making and support PCN staff and patients to be more prepared to shared decision-making conversations.
  • This post is a PCN role, so you may be required to work at other NN3 practices.

About us:

We are a busy and friendly practice with 3 Partners, 3 Salaried GPs, 2 Physicians Assistants, 4 Practice Nurses, 2 Assistant Practitioners and an excellent administration support team. We are based on the Norfolk Broads and operate from our main purpose-built premises in Coltishall and a smaller branch surgery in Spixworth. We have a largely rural based patient population approaching 9,000.

Job responsibilities:

  • Raise awareness of how to identify patients who may benefit from shared decision making and support PCN staff and patients to be more prepared to shared decision-making conversations.
  • Work in an administrative support role to the Nurse Manager and nursing team, to include the smooth-running of the recall system.
  • Contact patients for the PROACTIVE initiative, referring onwards as necessary.
  • Administration for weekly Care Hone clinical ward rounds.
  • Ardens diary recalls.
  • Smooth-running of recall systems for Learning Disabilities, SMI invites, eating disorders and NHS Health Checks.
  • Administrative management of smear results.
  • Administrative management of LARC.
  • Administrative management of vaccination invites and housebound patients.
  • Assist the Practice Manager and Business Manager to manage other clinical, LES, DES programmes.
  • Attend and participate in any practice meetings when required.
  • Undertake any other activities that may from time to time be reasonably requested by the Business Manager, the Nurse Manager, the Practice Manager or the Partners.

Person Specification:

  • Qualifications: High level of verbal, written and numerical literacy. Self-development through continuous professional development activity.
  • Skills & Abilities: Ability to communicate complex and sensitive information effectively with people at all levels by telephone, email and face to face. Ability to follow legal, ethical, professional and organisational policies/procedures and codes of conduct. Excellent interpersonal, influencing and negotiation skills, organisation skills with the ability to constructively challenge the view and practices of managers and clinicians. Ability to work under pressure and to meet deadlines. Good IT skills knowledge of all office programmes. Ability to maintain and understand the need for complete confidentiality and data protection. Ability to use own initiative, discretion, and sensitivity. Accuracy and excellent attention to detail. Full driving license. Knowledge of Emis.
  • Experience: Experience of working with healthcare professionals and or previous experience in the NHS or social care or relevant field. An appreciation of the nature of GPs and general practice. An appreciation of the nature of primary care prescribing. Experience and an awareness of common acute and long-term conditions that are likely to be seen in general practice. In-depth knowledge and understanding of the principles of evidence-based healthcare. Understanding of the implications of national priorities for the team and service. An appreciation of the new NHS landscape including the relationship between individual providers, PCNs and commissioners.
