We are looking for an experienced Stoma Nurse who has the passion, drive, and commitment to develop and deliver community Stoma Care services in collaboration with our partnership accounts in Staffordshire and Shropshire.
We want a nurse who can work as an independent practitioner, is self‑motivated and has a winning mindset. They need to be a strategic thinker who wants to build on their commercial awareness and skills and who has the confidence and courage to challenge.
The Role- Deliver professional and ethical care to patients
- Support the patient pathway from hospital to home, providing home visits and community clinics
- Effectively and efficiently plan, manage, and organise caseload and time
- Communicate effectively and provide feedback to patients / carers and referring Healthcare professionals
- Undertake an Appliance Use Review (AUR) where appropriate as per company and DoH Guidelines
- Maintain clinical records in accordance with Company, Legal and Professional requirements
- Instigate working practices to ensure collaborative working between different stakeholders e.g., acute and community health care teams
- Generate and maintain effective working relationships with health care professionals across the region
- Deliver educational support to fellow health care professionals
- Undertake service audit/evaluation activities
- Registered General Nurse
- Qualified for a Nursing Degree
- Expert clinician in Stoma management
- Sound interpersonal and relationship building skills
- Teaching experience
- IT proficiency
