We are looking for an experienced Nurse with strong clinical expertise, passion, and commitment to deliver high‑quality community care services across the GESH group, mainly the St Georges area. This role involves close collaboration with our acute partners and supporting patients within the surrounding community settings. The successful candidate will ideally be based in or around Wimbledon or Tooting, and both full and part time working requests will be considered. We are seeking a Nurse who is self‑driven, proactive, and solutions‑focused.

The Role

  • Deliver professional, compassionate, and ethical care to patients
  • Support the patient pathway from hospital to home through home visits and community clinics
  • Plan, manage, and organise caseload and time effectively and efficiently
  • Communicate clearly and respectfully with patients, carers, and referring healthcare professionals
  • Undertake Appliance Use Reviews (AURs) in accordance with company and Department of Health guidance
  • Maintain accurate clinical records in line with Company, Legal, and Professional standards
  • Support joined‑up working practices between acute teams, community services, and other stakeholders
  • Build and maintain effective working relationships with healthcare professionals across the GESH region
  • Deliver education and training to fellow healthcare professionals
  • Participate in service evaluation, audit, and development activities
  • Engage with ongoing training and development, including future upskilling in Peristeen

The Requirements

  • Registered General Nurse
  • Stoma Care experience is desirable
  • Strong interpersonal, communication, and relationship‑building skills
  • Experience in teaching or delivering education
  • Confident IT user
