We are seeking an experienced Nurse with relevant clinical skills, passion, drive, and commitment to develop and deliver community Stoma Care services in collaboration with our partners at Sheffield Teaching Hospitals. The ideal candidate can work independently, is self-motivated, and possesses a strategic mindset. They should aim to enhance their commercial awareness and skills, with the confidence and courage to challenge when necessary.
The Role
- Deliver professional and ethical care to patients
- Support patient pathways from hospital to home, including home visits and community clinics
- Plan, manage, and organise caseloads and time effectively
- Communicate effectively and provide feedback to patients, carers, and healthcare professionals
- Undertake Appliance Use Reviews (AUR) as appropriate, following company and DoH guidelines
- Maintain clinical records in line with company, legal, and professional standards
- Promote collaborative working practices among stakeholders, e.g., acute and community health care teams
- Build and maintain effective relationships with healthcare professionals across the region
- Provide educational support to healthcare professionals
- Participate in service audits and evaluations
