We are seeking an experienced Nurse with relevant clinical skills, passion, drive, and commitment to develop and deliver community Stoma Care services in collaboration with our partners at Sheffield Teaching Hospitals.

The ideal candidate is an independent practitioner who is self-motivated, strategic, confident, and courageous to challenge when necessary. They should aim to enhance their commercial awareness and skills.

The Role

1. Deliver professional and ethical care to patients.
2. Support patient pathways from hospital to home, including home visits and community clinics.
3. Plan, manage, and organize caseload and time effectively.
4. Communicate effectively and provide feedback to patients, carers, and healthcare professionals.
5. Undertake Appliance Use Review (AUR) as appropriate, following company and DoH guidelines.
6. Maintain clinical records in accordance with legal and professional standards.
7. Foster collaborative working practices among stakeholders such as acute and community health care teams.
8. Build and maintain effective relationships with healthcare professionals across the region.
9. Provide educational support to healthcare professionals.
10. Participate in service audits and evaluations.

The Requirements

• Registered General Nurse qualification.
• Nursing degree.
• Expertise in Stoma management.
• Strong interpersonal and relationship-building skills.
• Teaching experience.
