We are looking for a proactive, motivated and organised Care Coordinator to join our friendly GP practice team here at Colney Hatch Lane Surgery. This is an exciting opportunity to play a key role in improving patient outcomes by coordinating care, supporting preventative health initiatives, and helping patients access the services they need.
Working closely with GPs, clinical and administrative staff and external healthcare providers, you will support patients with long-term conditions, preventative healthcare programmes, and personalised care planning, while also providing essential administrative support to ensure the smooth running of the practice.
Main duties of the job
- Assisting with patient triage by directing patients to the most appropriate clinician or service.
- Contacting patients to arrange annual health and wellbeing reviews, particularly for those with long-term conditions and patients who are housebound.
- Providing day-to-day administrative support, including processing letters, updating patient records, responding to patient enquiries and helping out on Reception when required.
- Encouraging patients who have missed their cancer screening appointments to attend by contacting them and helping arrange appointments.
- Arranging referrals to hospitals and community services, keeping track of their progress and following up where necessary.
- Supporting our vaccination programmes, including coordinating COVID-19 vaccinations for housebound patients and assisting with vaccine clinics when needed.
- Keeping patient records accurate, up to date and confidential using the practice clinical system.
- Speaking with eligible patients about NHS Weight Management Programmes and helping them access the right support.
- Working closely with GPs, nurses, the wider practice team and external healthcare providers to deliver joined-up, patient-centred care.
- Chasing referral appointments, reports and outcomes to help ensure patients receive the care they need without unnecessary delays.
- Following all NHS guidance, practice policies and information governance requirements to ensure safe and effective working.

We are a friendly family-oriented GP practice located in Muswell Hill with a supportive multidisciplinary team consisting of 2 GP Partners, Practice Nurse, Administrative and reception team, and Practice Manager. We use EMIS Web, Accurx, and integrated AI-supported systems to support efficient clinical workflow and patient care. We operate a total triage model focused on safe, responsive, and accessible care for our patients. We pride ourselves on maintaining a positive working culture with strong teamwork and mutual support across all members of staff.
