CNWL are pleased to announce a vacancy within Hillingdon's Palliative care service. We are looking for an innovative, enthusiastic and dynamic nurse to join our team whose aim is to improve the coordination and continuity of care and support for patients and their families/carers. Enable supportive and proactive nursing care for people in the community who are palliative and at the end of life.
You should have a creative, imaginative and flexible approach to service delivery and quality improvement and have excellent interpersonal skills with post graduate experience of working in the community, hospital or hospice environment with associated palliative care experience. Flexible working hours are available. This is a Monday to Friday service working 8am to 4pm. Car user essential.
Responsibilities- Triaging telephone calls into the service from patients, carers, relatives and clinicians.
- Triaging referrals into the service.
- Enable supportive and proactive nursing care for people in the community who are palliative or at the end of life, providing high quality end of life care.
- Work within a multi-disciplinary team to ensure excellent communications are upheld.
- Hospital admission avoidance and uphold ACP wishes of patients, supporting them to achieve their desired place of care or death.
- Contribute to the development and implementation of uni-disciplinary and multi-disciplinary clinical policies, guidelines, protocols and care pathways.
- Participate in multi-disciplinary assessments and cross-agency assessments as required.
- Ensure dignity for patients and their families at all times and maintain clinical records.
- Provide specialist contribution to health promotion and health education initiatives within the team.
- Attend and actively participate in departmental and nursing team meetings; contribute to maintaining effective communication with the service and other relevant services.
- Attend, participate in, and lead multi-disciplinary meetings and case reviews as required; participate in educational programmes and training initiatives to improve palliative skills across professionals.
- Act as a mentor for nursing students undergoing CPD and as an educational resource for other nursing staff in managing patients and families with palliative care needs.
- Evaluate education and training initiatives and assist administration with up-to-date statistics on personal performance; develop skills and knowledge and attend accredited courses.
- Share specialist knowledge gained through study or accredited courses with others; train and disseminate to improve practice.
- Comply with all mandatory training and establish effective lines of communication within the team, with service users, and with other health agencies.
- Communicate sensitive information in a form understandable to patients and informal carers and in line with national/local guidance; use verbal and non-verbal communication tools to educate and advise patients and carers.
- Provide copies of all correspondence to patients according to Trust policies; advise and negotiate complex information exchange with staff, patients, carers and local agencies.
- Inform the Line Manager promptly of any complaints or concerns.
- Make effective presentations to internal and external staff groups for training and public relations purposes; contribute to research activities of the Palliative Care Team as required.
- Support research activity and recruitment where required; stay abreast of relevant clinical issues and research in palliative care.
- Ensure clinical practice aligns with national/local nursing guidelines and demonstrates clinical effectiveness in patient care; contribute to Clinical Governance strategy of the Palliative Care Centre and Trust.
- Synthesise data and evidence from published research to inform role, service and current practice.
- Post-graduate experience of working in the community, hospital or hospice environment with palliative care experience.
- First-class clinical skills with ability to triage clinical calls, strong leadership, and ability to work well in a changing, innovative environment.
- Ability to work autonomously in isolated community care settings and to operate within a general Multi-Disciplinary Team to advise on palliative approaches.
