If so, we would like to offer you an opportunity to do this with our Home Treatment Team in Older Adults Community Mental Health Service at the heart of Westminster. This is an opportunity for qualified nurses who are looking to develop and establish their skills and expand their abilities to work within the community. In turn, you will be able to make a meaningful impact in a different capacity than your existing role. You will need to demonstrate resilience and the ability to adapt to the changing landscape and opportunities.

Join our team at HTT, where your work-life balance is a priority! Our services run from 9 am to 5 pm, seven days a week, with no night shifts (some weekend work would be required). This structure not only ensures you have dedicated time for your personal life but also opens up opportunities for you to take on additional bank hours with us or explore roles elsewhere across the trust.

This role involves intensive support and treatment to patients in the community in the acute phase of a mental health crisis as an alternative to inpatient care; monitoring patients' safety and well-being to promote independence in the community; supporting early discharges from hospital.

To help you achieve expanding your knowledge we will offer you:

Please note that this role requires successful candidates to be able to engage in Therapeutic Management of Violence and Aggression (TMVA) training, including the use of force techniques and the ability to attend resuscitation training and perform CPR. This is because of the high-acuity nature of the ward/community environment and the need to ensure the safety of both patients and staff. The role is conditional upon candidates being able to engage in TMVA training and resus training during the course of their employment whilst undertaking this role.

Clinical duties include:

  • Triage referrals and allocate work to junior staff as appropriate.
  • Provide a comprehensive mental health assessment for service users with complex needs in the acute phase of a mental health crisis.
  • Assess current risks and identify how the risk and service users' mental health needs would best be met.
  • Formulate, implement and review individualised Recovery Care Plans with patients and their carers that aim to address their psychological, physical & social needs.
  • Build and maintain therapeutic relationships with service users and carers and involve them in drawing up care plans.
  • Work with service users' care co-ordinator/lead professional as appropriate to ensure continuity of care.
  • Provide information and advice to carers to support them through the crisis period.
  • Administer, supervise and monitor prescribed medication advising service users and carers on storage, administration and possible side effects.
