As a Co-ordinator, you'll play a crucial role in managing and leading care co-ordination across your designated patch of Redbridge. You will be working closely with the Field Care Supervisor, liaising with Local Authorities, obtaining referrals and taking on new packages. Your responsibilities will include:

  • Managing and overseeing daily rotas, ensuring efficient scheduling and continuity.
  • Maintaining meticulous documentation and ensuring compliance with CQC standards.
  • Processing local-authority packages and collaborating closely with field staff.
  • Handling escalations, concerns, and participating in the on-call rota.

Requirements:

  • Level 3 Health & Social Care qualification.
  • Minimum 2 years' experience as a Care Co-ordinator in domiciliary care.
  • Strong knowledge of Access Care Planner or similar ECM systems.
  • Proficiency with Excel and data management.
  • Full UK driving licence and access to a car.
