As a Co-ordinator, you'll play a crucial role in managing and leading care co-ordination across your designated patch of Redbridge. You will be working closely with the Field Care Supervisor, liaising with Local Authorities, obtaining referrals and taking on new packages. You will manage and oversee daily rotas, ensuring efficient scheduling and continuity. Maintain meticulous documentation and ensure compliance with CQC standards. Process local-authority packages and collaborate closely with field staff. Handle escalations, concerns, and participate in the on-call rota.
Requirements:
- Level 3 Health & Social Care qualification
- Minimum 2 years' experience as a Care Co-ordinator in domiciliary care
- Strong knowledge of Access Care Planner or similar ECM systems
- Proficiency with Excel and data management
- Full UK driving licence and access to a car
