You will be working closely with the Field Care Supervisor, liaising with Local Authorities, obtaining referrals and taking on new packages. Your responsibilities will include:
- Managing and overseeing daily rotas, ensuring efficient scheduling and continuity.
- Maintaining meticulous documentation and ensuring compliance with CQC standards.
- Processing local-authority packages and collaborating closely with field staff.
- Handling escalations, concerns, and participating in the on-call rota.
Requirements:
- Level 3 Health & Social Care qualification.
- Minimum 2 years' experience as a Care Co-ordinator in domiciliary care.
- Strong knowledge of Access Care Planner or similar ECM systems.
- Proficiency with Excel and data management.
- Full UK driving licence and access to a car.
