Veterans in Communities is recruiting a VALOUR Pathways Coordinator to support delivery of the VALOUR North West programme.
This role provides person-centred support to veterans and their families, ensuring they can access the right services at the right time.
Working as part of a coordinated regional network, you will deliver support through a structured Pathways Coordination approach, including triage, assessment, case management and referral.
You will play a key role in ensuring individuals receive consistent, joined-up support in line with the VALOUR "No Wrong Front Door" model.
Responsibilities
- Provide person-centred support to veterans and their families.
- Help individuals access the right services at the right time.
- Work as part of a coordinated regional network to deliver support through the VALOUR North West Pathways Coordination approach.
- Carry out triage, assessment, case management and referrals as part of the coordinated pathway.
