Job overview: We are looking for a dynamic, enthusiastic and highly motivated dietitian to join CNWL to work in the Community Adult Rehabilitation Service in Hillingdon. You will be a part of a multidisciplinary team, working closely with Physiotherapists, Occupational Therapists, Speech and Language Therapists, Rehab Support Workers and wider community MDT to provide quality care to patients in their own homes. This post offers the opportunity to gain clinical experience in community oral nutritional support and exposure to specialist Home enteral feeding service. The role includes an opportunity to work with patients with a variety of neurological conditions. The post holder will be supported by B7 Dietitian and wider Community MDT. Continuing professional development is actively supported via clinical supervision, close liaison with local acute dietetic services and other stakeholders, and in-service training.

Main duties of the job:

  • Work autonomously to provide dietetic advice to patients, their carers and staff with a range of medical conditions in service users home or intermediate care unit within agreed protocols and guidelines.
  • Carry out nutritional assessments to determine the nutritional problem.
  • Calculation of nutritional requirements, development of treatment plans, client centred goals and discharge planning including patients on artificial nutrition support.
  • Prescription of dietary treatment and ACBS products for conditions where there are limited professional/clinical guidelines available.
  • To be clinically competent to give dietetic advice, treatment and dietary counselling to patients in their home and intermediate care or group settings.
  • Independently interpret complex clinical information and use professional judgement to adapt nutrition care plans in response to changing clinical conditions.
  • Assess enteral feeding sites, identify clinical risks, and make appropriate treatment recommendations.
  • Participate in the health promotion activities of the department.
  • Adhere to professional standards, clinical guidelines, and confidentiality requirements.

Qualifications:

  • Registered member of the Health and Care Professionals Council.
  • Member of the British Dietetic Association.

Experience:

  • Broad range of learner placements or community experience in the NHS.
  • Experience in oral nutrition support and home enteral feeding practices.
  • Experience in nutritional screening training to other health professionals.
  • Experience of multi-disciplinary team working.
  • Evidence of developing, implementing and evaluating projects.
  • Evidence of delivering training sessions.
  • Experience of clinical audit.
  • Experience of teaching and assessing students.

Skills and Abilities:

  • Ability to demonstrate verbal communication and presentation skills to professional staff and the general public.
  • Ability to generate written communication that is relevant, concise, accurate and legible.
  • Ability to work within a multicultural setting.
  • A demonstrable understanding of dietetics within a community setting.
  • Demonstrate competence to work both as an autonomous practitioner, and within a multidisciplinary setting.
