Camden Rapid Access Services (RAS) is a physical health admission avoidance and supported early discharge service with three pathways (Discharge to Assess, Urgent community Response, Virtual ward). The role will work across all pathways.
Applicants must be a qualified ACP with experience in acute care (e.g., A&E, AMU, ITU, Virtual Ward, Paramedic), completed the ACP modules, and be a Non‑medical prescriber V300.
Main duties of the job
Shifts: Monday–Friday 9am–5pm (potentially change with a seven‑day service). Flexible working is offered.
Lone working: The role requires independent work in patients’ homes or community settings, including evenings.
Other responsibilities include: managing clinical care autonomously, developing treatment plans, collaborating with multidisciplinary teams, leading clinical pathway integration, and ensuring high‑quality person‑centred care.

Detailed job description and main responsibilities
The role requires a high level of autonomy and complex decision‑making, supported by a master’s level qualification in clinical practice, leadership, management, education, and research. The post holder will manage care in partnership with individuals, families, and carers, analysing complex problems to devise innovative solutions that enhance outcomes.
Key responsibilities include delivering advanced practice autonomously in general medicine, frailty, respiratory, and cardiology, supporting patients as alternatives to hospital admission and early supported discharge, and integrating care pathways across community and acute settings.
Applicants must hold current professional registration (NMC or HCPC), a recognised MSc in Advanced Clinical Practice with ACP status, and an Independent Prescribing qualification (V300 or equivalent). Trainees and those currently in training will not be considered.

- MSc, Advanced Clinical Practice.
- Registered nurse (NMC) or other allied healthcare professional (HCPC).
- Non‑Medical Prescriber (V300).
- Evidence of ongoing professional development.
Experience
- Expert knowledge and experience in acute and/or community settings.
- Autonomous practice as an ACP, managing patients with undiagnosed conditions, investigations, diagnostics, and management plans.
- Involvement in developing care programs, protocols, clinical audit, and service improvement.
- Contribution to the Trust’s clinical governance objectives.
