An exciting opportunity has arisen for a multiprofessional Advanced Clinical Practitioner to join the Home 1st Unplanned Care Service within Milton Keynes Community Health Services. This role directly supports national priorities to strengthen hospital‑to‑home pathways by expanding community‑based care and reducing pressure on acute services, and to shift care closer to home through enhanced community provision and reduced reliance on hospital admission.

Detailed job description and main responsibilities:

  • Provide clinical cover across the inpatient pathway – Windsor Intermediate Care Unit (WICU) and Seacole Community Hospital – and support Virtual Ward and Urgent Community Response, with a strong focus on admission avoidance.
  • Assess, diagnose, plan, implement and evaluate treatments and interventions for patients with complex, undifferentiated or undefined presentations.
  • Perform clinical procedures appropriate to history and examination, including invasive tests and precision treatments where indicated.
  • Request investigations (blood, urine, lab tests, ECGs, ultrasound, X‑rays, CT scans) in accordance with local policy.
  • Use clinical reasoning and decision‑making skills to make differential diagnoses and provide rationales for management plans, continuously reflecting and evaluating practice.
  • Prioritise health problems and intervene appropriately in complex, deteriorating, urgent and emergency situations, including initiation and leadership of resuscitation.
  • Manage clinical events requiring unpredictable and high levels of physical effort, depending on patient dependency and clinical need.
  • Act as a specialist resource to all staff in the delivery of clinical care to patients.
  • Advocate high‑quality care for people, representing and supporting the person’s interests at team meetings and during hospital and home care.
  • Empower patients and families through appropriate support and education.
  • Influence and support public health/self‑care initiatives for individuals and the community.
  • Collect, collate, evaluate and report clinical information, maintaining accurate patient records related to assessment and care planning.
  • Develop clinical protocols and guidelines within own area of practice to ensure quality of care.
  • Prescribe and review medication as an independent prescriber where appropriate, within legal framework and relevant protocols; liaise with pharmacy and medical teams regarding any changes.
  • Deliver psychological therapies appropriate to patient need and within the scope of practice and supervision framework.
  • Regularly evaluate practice to ensure continuous improvement, working with clinicians and management to examine episodes of care and improve services.
  • Enable effective and comprehensive admission, transfer and discharge of care where indicated in scope.
  • Ensure implementation, monitoring and training of infection control and health and safety policies and procedures.
  • Participate in audits and research, and engage in strategic responsibilities for clinical pathways or education as indicated.

Qualifications:

  • MSc/PG Dip in Advanced Clinical Practice or equivalent.
  • Current registration with the Nursing and Midwifery Council (NMC) or Health and Care Professionals Council (HCPC).
  • Additional leadership qualifications.
  • Valid V300 licence or equivalent.

Experience:

  • Leadership experience at B7 level or above.
  • Proven experience as an independent practitioner.
  • Experience of leading, planning and completing audit or research projects.

Personal Disposition:

  • Self‑motivated and innovative.
  • Ability to operate under challenging working conditions.
  • Car for business use.
