  • Salary: £45,953 – £54,254 per annum (incl. Outer HCAS – pro rata if part‑time)

Key Responsibilities

  • Function fully within the Mental Health In‑reach Team, assessing and treating patients throughout their journey in prison.
  • Accept referrals and triage patients with the team.
  • Undertake Early Days in Custody (EDIC) assessments and ensure all new arrivals receive a brief assessment of individual presentation and need.
  • Participate in Care and Separation (Segregation) Unit (CSU) rounds with a GP and primary care nurse.
  • Carry out comprehensive assessments with or without colleagues and provide feedback to the multi‑disciplinary team.
  • Provide effective case management to a defined caseload in accordance with the Care Programme Approach (CPA), coordinating with prison departments and external health, social care and probation services.
  • Ensure effective pre‑release care planning and facilitate smooth transition to community or alternative prison settings.
  • Detailed Responsibilities
  • Promote the health and well‑being of patients in a safe, caring and therapeutic environment based on an individualised approach.
  • Work with the Team Leader to provide an environment that promotes training and development among the workforce.
  • Work under the direction of the team manager to ensure clinical priorities are clearly understood and met to an agreed standard.
  • Assess health and social care needs and negotiate care plans with patients and staff teams.
  • Undertake risk assessments and formulate comprehensive plans developed jointly with patients and staff teams.
  • Operate within the Prison/NOMS policies and the safer custody framework for managing suicide and self‑harm (ACCT procedures).
  • Provide a service sensitive to age, culture, ethnicity, gender, sexuality and disability.
  • Contribute to effective multi‑disciplinary team working within the prison.
  • Communicate effectively with relevant internal departments and external agencies (within the restrictions of a prison environment).
  • Offer a range of evidence‑based interventions and proactive risk‑management approaches to reduce and manage symptoms and high‑risk behaviours.
  • Ensure safeguarding procedures are followed and monitor treatment, supporting concordance and timely action to alleviate unwanted effects.
  • Provide written reports, discharge summaries and coordinate CPA review meetings, producing written care plans and circulating them to relevant parties.
  • Have a working knowledge of the MHA 1983 (Amendment 2007) as used within criminal justice settings.
  • Plan patient care transition back into the community from prison, ensuring continuity of care.
  • Ensure the service is delivered sofully and effectively, inclusively and consistently.

Qualifications & Experience

  • Registered Practitioner (Registered Mental Health Nurse).
  • Registered
  • Evidence of professional development since registration.
  • Post‑registration qualification that enhances clinical work (e. g., CBT, DBT, Dual Diagnosis).
  • Experience of working as part of a multi‑disciplinary team and within the CPA framework.
  • Experience of care coordinating within a community setting.
  • Experience of mentoring or supervising students.
  • Experience of working in a prison or forensic environment.
