Salary

£45,953 – £54,254 per annum (inclusive of HCAS).

Contract

Permanent, full‑time, 37.5hrs per week, Band 6 (NHS Af C).

Responsibilities

• Assess service users’ mental and physical health status and needs.
• Assess other needs related to health and wellbeing.
• Undertake and input into risk assessments jointly with the MDT, and develop robust risk‑management plans in line with Trust policy.
• Develop and deliver robust care plans in partnership with multidisciplinary colleagues and a range of health and social care agencies, within the DIALOG+ framework.
• Identify how best needs can be met and facilitate sign‑posting, onward referral or rapid access to relevant professionals and services; provide support and advice to GP on effective management.
• Provide support and advice to service users, carers and other professionals on mental health issues.
• Deliver high‑quality, person‑centred care within available resources, ensuring compliance with Trust policy.

Qualifications

• Professional qualification in Mental Health Nursing, Occupational Therapy or Social Work.
• Post‑basic education evidence.

Skills & Abilities

• Participation in MDT work and contribution to Hub activities.
• Knowledge of therapeutic assessment and interventions (risk assessment, MSE, capacity assessment, initial assessments) to reduce distress for individuals with mental health issues.
• Ability to support individuals experiencing acute mental illness distress and crisis via care‑planning and safety‑planning.
• Understanding of legislation relevant to mental health practice.
• Prioritisation of competing demands.
• Reflective practice and interpersonal relationship skills.
• Effective verbal, non‑verbal, and telephone communication.
• Proficiency in IT: Microsoft Office and Systm One Database.
• Capacity to teach and supervise qualified and unqualified staff.
• Skills in a range of therapeutic interventions, psycho‑social treatment options.

Experience

• Minimum 2 years post‑registration experience in mental health nursing, occupational therapy or social work.
• Experience working within a community setting.
• Experience supervising the work of others.
• Knowledge & Understanding
