We are recruiting a Band 5 Community Registered Nurse for the East Integrated Neighbourhood Team at Kentish Town Health Centre in Camden.
A Band 5 nurse will deliver community nursing care within a skill‐mixed, seven‐day-a‐week team, supporting patients in their own homes and across Camden. The role is part time – 26.25 hours per week – and is a fixed‐term contract of 12 months, starting 1 September 2026.
Main duties
- Providing high‐quality nursing interventions to housebound patients, including holistic assessments and care plan implementation.
- Managing a designated caseload and supporting junior staff and students.
- Working closely with multidisciplinary teams – nurses, therapy staff, pharmacists, social care, mental health, voluntary sector and general practice.
- Collaborating with specialist services such as Rapid Response, Palliative Care, Tissue Viability, Bladder & Bowel, Stroke, Respiratory, Heart Failure and Diabetes teams.
- Ensuring safe clinical practice through assessments, documentation and adherence to mandatory training.
- Participating in practice development, quality improvement and supervisory roles.

Qualifications
- NMC Registered Nurse (Adult) or awaiting PIN, diploma or degree.
- Numeracy skills required for safe medication administration.
- Evidence of ongoing relevant CPD.
- Community experience as a student or substantive role preferred.
Skills and knowledge
- Excellent communication with multidisciplinary teams and patients.
- Proficiency in electronic patient records and Microsoft Office.
- Understanding of the Mental Capacity Act and Deprivation of Liberty Safeguards.
- Ability to work autonomously, organise workload and supervise support workers.
- Knowledge of quality improvement principles and recent health‐care developments.
