Overview CNWL NHS Foundation Trust is seeking a dedicated Band 6 Nurse to join our Community Children\’s Nursing Team in Ealing. We are looking for a full-time (37.5 hours per week) post. Our team provides compassionate, family-centered nursing care to children with acute, complex, and long-term health conditions, including palliative and end-of-life care, in the community and hospital settings. We welcome applicants with Band 5 experience or current Band 6. Experience in the community is preferred but not essential. The Children\’s Community Nursing Team provides clinical nursing care, training and support to children, young people (0-19) and their carers at home or other community settings, enabling them to remain safely in the community and transition to adult services where appropriate. They are expected to be kind, responsive, professional and informative, contributing to service quality by:Using clinical skills and risk assessments to keep service users safeFollowing evidence-based policies to achieve the best clinical outcomesRespecting and empowering people in care planningAs part of a multidisciplinary team, you will supervise others and contribute to quality improvement and safety. You will act as a role model and support band 5 nurses, students, and other staff to ensure standards are met.Key ResponsibilitiesCommunity Children\’s nurses contribute to safe and reliable services by:Applying clinical judgment and risk assessments to ensure safetySafeguarding and escalating concerns as neededAdvocating for patients and maintaining safety in practiceSupervising others and reflecting on practice to improve safety and qualityMaintaining mandatory training and participating in continuous improvementThe NMC Code of Conduct applies and nurses must register with the NMC annually.Clinical OutcomesCommunity Children\’s nurses work with complex needs and contribute to outcomes by:Being autonomous but collaborative in a multidisciplinary teamProviding evidence-based, policy-adherent nursing careWorking with patients and families through care planning, including risk assessmentAdvocating for patients and overseeing junior staffBalancing autonomy with teamworkContributing to high-performing teams by:Effective communication across the teamClarifying roles and encouraging reflective practiceLearning from adverse events and data to drive improvementSupporting patient empowerment by:Partnering with patients and families in decision-makingGaining informed consent and guiding patients to available courses and servicesMaintaining professionalism and a positive, inclusive cultureSupporting yourself and othersAll services are expected to meet Care Quality Commission standards.
