CNWL NHS Foundation Trust is recruiting for a Band 6 Full Time Assertive Outreach Worker (Nurse / OT / Social Worker) within the Community Mental Health Hub in Brent.

The role focuses on outreach to adults with severe mental illness to stabilise engagement and prevent crises.

You will be a trusted keyworker, delivering intensive support across Brent in home and community settings, coordinating with CMHH MDT and external services, and supporting transition back to routine CMHH care.
