Job Overview

Are you an innovative & motivated Occupational Therapist ready to work in the Ealing Community Short‑Term Rehabilitation service? We have an exciting opportunity for a Band 5 Occupational Therapist to join our short‑term rehab team, focused on helping patients improve their abilities and achieve rehabilitation goals through therapy‑led care. The role emphasizes achieving goals and enabling people to regain independence and social integration effectively after discharge from acute hospitals and community services in the borough.

Salary: £35,763 – £43,466 per annum (inc HCAS; pro‑rata if part‑time)

Grade: Band 5

Contract: Permanent

Hours: 37.5 hours per week (weekend working required)

Working pattern: Full time, flexible working, bank assignments available

Main duties of the job

• Provide specialised rehabilitation assessment, education, advice and treatment to adult patients with complex health and social care needs in primary care.
• Work in a variety of community settings including patients’ homes, intermediate care wards, nursing homes and residential homes, delivering intensive rehabilitation.
• Independently manage a defined caseload of complex cases, contributing to the provision of a comprehensive rehabilitation service.
• Undertake seven‑day working as required by the service.

Additional responsibilities

Work cohesively within the team and support the development of the service through active teamwork and shared learning. Provide written and verbal reports or recommendations to referrers, patients and other agencies as required, ensuring coordination across multidisciplinary meetings.

Person specification

Skills and knowledge

• Complete a comprehensive assessment for patients at risk of falls.
• Demonstrate competence in using computers, including internet, Word, PowerPoint, Excel and electronic patient records.
• Awareness of current health issues, including clinical governance and its impact on team practice.

Experience

• Experience in community settings with older people.
• Undergraduate experience in a variety of core clinical settings.
• Experience in the UK health service as an Occupational Therapist, Physiotherapist, student or related role.
• Significant post‑registration work as Band 5 or equivalent in physical disability, community rehabilitation with an elderly population.

Other

• Valid driving licence and car ownership, or ability to drive pooled cars.

Education and qualifications

• State registration with the UK Health Professions Council.
• Degree or diploma in Occupational Therapy (or equivalent).
• Membership and/or involvement in a national special interest group.
• Post‑graduate training, in‑service or clinical placement training related to falls prevention, physical disability or community rehabilitation with an elderly population.
