Overview

Are you an innovative, organised, enthusiastic and motivated occupational therapist with a strong interest in neurology? We have an exciting Band 6 occupational therapist opportunity to join our specialist multidisciplinary community neuro rehabilitation service. The post includes the stroke pathway for clients recently discharged from stroke units and the long term neurological conditions pathway for progressive or acquired neurological conditions. You will develop your occupational therapy practice and deliver evidence-based assessment and rehabilitation with a focus on complex caseload management, time management and liaison. We will support you in developing your clinical, supervisory and management skills.

The Ealing Enable team is a valued part of the Maximising Independence team within Ealing Community Partners. Team members are based from either EDTC in Southall or Clayponds Hospital in South Ealing, with flexible working supported. There is a Clinical Lead (Band 8A) and a Band 7 Occupational Therapist to support development. Regular CPD programmes and supervision structures are in place. This is a team with career progression opportunities. This is an Ealing Community Partners role through Central North West London (CNWL) Trust.

1. To be professionally and legally accountable for all clinical work undertaken. To work within the standards and guidelines of the team, the Trust, and professional organisations.

Responsibilities

1. To undertake a range of routine and specialist assessments as an autonomous practitioner (individual or group) which is evidence based and uses standardised assessments and outcome measures where appropriate.
2. To have a comprehensive knowledge of anatomy, neuro-anatomy, and links with impairments and how they can impact on function; to educate others when required (e.g. post-stroke impairments and impact on function).
3. To use clinical reasoning to interpret and analyse clinical and non-clinical facts to form an accurate diagnosis and, with patient involvement, determine an appropriate care plan based on current professional opinion, prognosis, priorities and other health issues.
4. To formulate and negotiate patient-centred goals/clinical objectives with clients, carers/relatives and other health professionals and ensure these are reviewed and modified as appropriate.
5. To deliver evidence-based treatment programmes within a team approach in a variety of settings ensuring that manual handling, therapeutic handling, patient education, exercise prescription and self-management resources comply with current guidelines and are communicated appropriately.
6. To devise and manage a treatment plan tailored to the patient’s presenting condition, including: functional rehabilitation (ADLs & PADLs), vocational rehabilitation, equipment provision, orthotic assessment, community-based activities, cognitive/memory assessment and treatment, education, health promotion, psychosocial issues, and other treatment options.
7. To work collaboratively within a multidisciplinary team and support sign-posting activities across multiple statutory and voluntary organisations (e.g., referrals for investigations, team referrals, and referrals to community services).
8. To manage a caseload of clients with complex needs with supervision from senior staff; monitor, evaluate and review therapy within a multi-disciplinary framework and take a key worker role as indicated.
9. To ensure that the client understands the clinical reasoning process and management plan and is supported to engage in self-management activities where possible.
10. Act as an advocate on behalf of clients and carers by adopting a client-centred approach to support meaningful participation in life roles within Ealing.
11. To advise and teach carers/health workers to participate in ongoing therapy/rehabilitation care programmes for clients, as appropriate.
12. To modify the client’s therapy treatment/management plan following ongoing re-assessment, risk assessment and evaluation of progress.
13. To support discharge planning activities and risk assessments, including follow-up therapy recommendations, equipment needs and determining care needs after input has concluded.
14. To use knowledge of other multi-disciplinary roles and external agencies to ensure appropriate referrals are made for seamless transfer of care and to enable patient self-management.
15. To represent the occupational therapy service at multi-disciplinary team meetings, providing information on clinical progress, risk assessments and contributing to transfer of care decisions.
16. To actively participate in health promotion and health education initiatives in the locality.
17. To manage clinical risk within own and team caseload and delegate to rehabilitation assistants with appropriate supervision from senior staff.
18. To be confident in the safe use and application of all equipment used in patient care and rehabilitation, including specialist equipment.
