Overview
The successful candidate will support the DN Team Leader with leadership initiatives including caseload and staff management within the District Nurse Service structure. Depending on demand, the post holder will support new initiatives including post-Covid business as usual. You will also complete home visits to provide nursing care, as well as complex patient management and support. The post requires working weekends on a rotational basis to coordinate weekend working across the locality. There is 1 x 9 month secondment Band 6 Deputy District Nurse opportunity within the Hillingdon District Nursing team.

Responsibilities
- Take delegated responsibility for the assessment, management and evaluation of evidence-based nursing care to patients in a variety of community settings.
- Lead and motivate a team of community staff nurses and Health Care Support Workers; line management by the District Nurse Team Leader.
- Visit various locations throughout the supported area; hold a full UK driver’s licence and have use of a car for business purposes or have the ability to commute within the Trust’s geographical area with access to transport to deliver timely patient care.
- Work in line with CNWL Trust values, policies and guidance; encourage partnership working with neighbouring hospitals, Local Authorities and other providers; align with the Trust Vision and key priorities.
- Provide access to continuing professional development resources and receive regular supervision.
- Be a clinical leader, taking responsibility for patient safety, safeguarding, escalation of safety concerns and acting as an advocate for patients; supervise the work of others and reflect on practice to drive improvements in safety and quality.
- Maintain accurate, legible, comprehensive records and comply with mandatory training requirements.
Clinical Responsibilities and Outcomes
- Use clinical judgement and risk assessments to keep patients safe; contribute to safe, effective care for those with complex needs.
- Oversee the work of others, act as autonomous practitioner within the team, and participate in care planning including risk assessment and decision-making with patients and families.
- Contribute to high-performing teams through effective communication, role clarity, reflective practice, learning from adverse events, and implementing sustainable improvements in clinical outcomes.
Patient Experience
- Work in partnership with patients, families and carers; gain consent and involve people in decision making where possible.
- Signpost to courses and services provided by the Recovery and Wellbeing College, local authority or voluntary sector.
- Demonstrate professionalism, responsiveness, knowledge and confidence; challenge poor behavior and promote a positive, inclusive culture aligned with trust values.
- Respond to complaints or concerns in line with Trust policy.
Support for Staff and Quality Standards
- Demonstrate how CNWL and CQC standards are met; gather and record evidence, assess team performance, and contribute to quality improvement initiatives.
- Contribute to team performance management and maintain a positive culture for learning from mistakes.
- Deputise for the team leader/ward manager when necessary, including managing the team and coordinating shifts.
- Provide supervision, annual appraisals, and constructive feedback; support professional development and use CPD funds to develop clinical specialisms or leadership/management modules.
Qualifications and Requirements
The position requires that candidates hold a full UK driver’s licence and have access to suitable transport; willingness to travel within the Trust’s area is essential. The role involves contact with patients or service users and aligns with NHS vaccination guidance.
