Job summary
An exciting opportunity has arisen for a Staff Nurse to join Alwinton, part of Secure Care Services at Sycamore, Northgate Park.
Alwinton is a medium-secure inpatient service for men with learning disabilities who require specialist assessment, treatment and support.
Working within the nursing team and wider multidisciplinary team, you will deliver safe, effective and person-centred care. You will build therapeutic relationships, support patients to manage their needs and help them work towards recovery, improved quality of life and progression through their care pathway.
The role involves care planning, risk assessment, medication management, physical health monitoring and implementing individualised treatment plans. You will promote positive behaviour support, meaningful activities and therapeutic interventions to encourage engagement and reduce the risk of incidents.
You will need to remain calm and professional when responding to distress and behaviours of concern, recognise changes in presentation and communicate concerns promptly to maintain patient and staff safety.
We are looking for a compassionate, resilient nurse who is committed to improving outcomes for people with learning disabilities, communicates effectively and works well within a multidisciplinary team. This is an opportunity to develop specialist skills in secure learning disability care while contributing to a service focused on high-quality care, recovery and meaningful progress for patients.
Main duties of the job
Assess, plan, deliver and evaluate individualised nursing care for patients with learning disabilities and complex mental, physical and behavioural needs.
Build therapeutic relationships, promoting dignity, respect, choice, engagement and recovery.
Complete and review clinical assessments, risk assessments and care plans to reflect changing needs and risks.
Monitor mental and physical health, recognise changes in presentation and escalate concerns promptly.
Administer medication safely in line with Trust policies and professional standards.
Implement positive behaviour support strategies to reduce distress, prevent escalation and minimise restrictive practices.
Maintain a safe, therapeutic ward environment, balancing individual needs with medium-secure requirements.
Work with the multidisciplinary team, families, carers and external agencies to coordinate care and support progression through the care pathway.
Encourage meaningful activities, daily living skills and progress towards individual recovery goals.
Maintain accurate and confidential clinical records, including care plans, risk assessments, incident reports and handovers.
Safeguard patients and respond appropriately to concerns about abuse, neglect, exploitation or patient-to-patient incidents.
Contribute to shift coordination, delegation and support for healthcare support workers and nursing colleagues.
Participate in clinical reviews, handovers, supervision, reflective practice and team meetings.
