An exciting opportunity has arisen for a Community Practitioner to join our new Intensive Support Team (IST) working alongside the Community Autism and Learning Disability (Needs‑Led) Service.
Key Responsibilities
- Support people who present with behaviours that challenge the system.
- Work with people with complex mental‑health presentations who cannot be supported within community mental health teams due to their learning disabilities and/or autism.
- Provide support in the person's own residence to reduce or prevent the need for restrictive practices, inpatient services and out‑of‑area or residential placements.
- Deliver support 7 days a week, with core hours of 8 am to 8 pm and flexibility to provide care outside these hours when required.
- Integrate across all relevant services to deliver psychological and psychiatric person‑centred interventions for individuals accessing the specialist learning‑disabilities team or the enhanced primary care service.
- Act on early warning signs, deliver evidence‑based interventions, provide urgent and planned support, proactively target people with autism and/or learning disability at risk of unplanned mental‑health admissions, and support families/carers.
- Play a key role in planning transitions between settings, advocate for the person's rights, and ensure they are actively involved in decision‑making.
Qualifications & Experience
- Full‑time, band 6 post.
- At least 12 months post‑qualification experience in a registered field, preferably within autism or learning disability.
- Adaptable, able to work well under pressure with strong decision‑making skills and a positive attitude.
- Excellent communication skills and an understanding of the complexities of learning disability, mental‑health and autism.
- Mobility and geographical requirements: able to travel within South Tyneside and to other CNTW and STSFT sites for community appointments.
