The Personality Disorder Hub are recruiting for an Advanced Practitioner. This is an exciting opportunity for an experienced practitioner who is interested in further developing their specialist skills. As an Advanced Practitioner you will provide advanced clinical assessment, formulation, care co-ordination and therapeutic interventions for people who present with complex emotional, relational and personality difficulties combined with high risk to self.

The role will have a focus on co-ordinating complex, multi-agency care needs and supporting formulation-based clinical decision-making. The Advanced Practitioner will hold a relatively small care co-ordination caseload, taking responsibility for co-ordinating and integrating different elements of a person's care and ensuring that psychological formulation informs clinical decision-making, risk management and therapeutic intervention.

You will work as part of the multidisciplinary team to ensure that care plans are holistic, effective, goal orientated and person centred.

As a team we understand the importance of wellbeing and building resilience. We have a strong reflective culture, providing protected time and forums for learning, growth and development.

Although the post will be primarily based within the South Tyneside and Sunderland locality, the team operates across the North-East region, therefore, there will be a requirement to travel across the region as part of this role.

Main duties of the job

  • You will work closely with relevant professionals and agencies, co-ordinating input from relevant services and ensuring the overall care plan remains coherent, formulation-based, goal-oriented and person centered.
  • The focus of your input will be on providing safety, stabilisation, individual and wider system containment and the delivery of therapeutic interventions designed to support greater emotional and relational stability.
  • Your practice will be guided by providing adapted forms of Structured Clinical Management (SCM), trauma stabilisation, Dialectical Behaviour therapy (DBT) and Mentalisation Based Therapy (MBT) during the most unstable phase of a service user's care pathway.
  • There will also be the opportunity to provide scaffolding to a wide range of community teams, inpatient staff and third sector providers.
