Care Coordinator

CN HEALTHCARE is looking for an experienced, dedicated, and dynamic individual to undertake the role of Care Coordinator, coordinating care services for individuals in their own homes.

PURPOSE OF THE POSITION

1. To ensure the delivery of a high quality of service to clients by line managing care workers.
2. To establish a caring environment for service users through high standards of professional practice and to promote dignity, respect, and self-esteem for each individual.
3. To ensure that the requirements of the National Minimum Standards for Domiciliary Care agencies and that CQC regulations are adhered to.

DUTIES AND RESPONSIBILITIES

As a Care Coordinator, your responsibilities will be as follows:

1. Ensure the service users receive care and support as agreed and ensure that all calls are covered within your designated area.
2. To assist in the planning and implementing of care packages for individual homecare service users.
3. To manage and delegate the allocation of support workers to specific service users in line with their individual needs and the carer’s experience/expertise.
4. Ensure homecare workers’ availability and capacity is fulfilled.
5. To ensure that all the support workers who work with the new referrals are fully briefed to enable them to carry out their role effectively.
6. To represent the company in a professional manner at all times, on the telephone, face to face or in written communication.
7. To participate in staff training/development when this is deemed appropriate.
8. To maintain confidentiality at all times and carry out the company’s confidentiality policy.
9. To carry out on-call duties.

PERSON SPECIFICATION

1. Minimum ONE YEAR’S EXPERIENCE as a Care Coordinator for domiciliary care services, coordinating, scheduling, and supervising staff to meet clients\’ needs.
2. Understanding of principles of good care and CQC regulations.
3. Good IT skills, including email and Microsoft.
4. Flexible, willing to work outside normal office hours when required.
5. Good Team Worker.
6. Self-Motivated with the ability to work under own initiative and prioritise workload.
7. Driver with a full UK Driving Licence.
