The Clinical Lead in Community Care provides strategic and clinical leadership across community-based services, ensuring safe, inclusive, and person-centred care. This role is pivotal in shaping responsive care pathways, empowering frontline teams, and embedding service user voice in every aspect of delivery. Ideal applicants will be covering the East of England area, covering complex care packages across Norfolk, Suffolk and Essex.

Clinical Governance & Quality Assurance

  • Lead clinical oversight of community care services, ensuring compliance with CQC and safeguarding standards.
  • Drive continuous improvement through audits, reflective practice, and co-produced quality frameworks.
  • Embed trauma-informed, inclusive approaches that reflect the lived experiences of service users.

Team Leadership & Development

  • Supervise and mentor nurses and support workers.
  • Facilitate training, reflective supervision, and professional development aligned with care-led values.
  • Foster psychologically safe environments that support staff wellbeing and collaborative problem-solving.

Service Design & Transformation

  • Co-design care pathways with service users, families, and stakeholders to address social isolation and promote inclusion.
  • Analyse data and feedback to inform strategic planning and resource allocation.

Stakeholder Engagement

  • Build partnerships with local authorities, voluntary sector organisations, and advocacy groups.
  • Represent the service in strategic forums, promoting care-led leadership and service user voice.
  • Champion inclusive communication and accessible service information.

Qualifications & Experience

  • Registered Nurse with active registration.
  • Significant experience in community care, clinical leadership, and service development.
  • Strong understanding of safeguarding, inclusion, and co-production principles.
  • Proven ability to lead dispersed teams and navigate complex care systems.
  • Full UK driving license.

Skills & Attributes

  • Empathetic, strategic, and results-oriented.
  • Skilled in stakeholder engagement and communication.
  • Adaptable and proactive in troubleshooting technical and operational barriers.
  • Flexible approach to travel and supporting the business needs.
  • Complex Care experience in the community which includes Airway Management and other additional health needs that include nurse delegated tasks, such as enteral feeding, suprapubic catheter care, spinal care and degenerative conditions.
