The Woodberry Practice is seeking an enthusiastic and experienced Clinical Pharmacist to join our primary care team. This face-to-face role works directly with patients and alongside GPs and the wider multidisciplinary team to optimise medicines and support high-quality patient care.
You will undertake structured medication reviews, support patients with long-term conditions, and assist with medicines reconciliation after hospital discharge.
