About the Role:
- We are seeking a highly motivated and compassionate Social Worker to provide specialist support and advocacy for adults with learning disabilities and autism. In this vital role, you will be instrumental in:
- Conducting comprehensive assessments of needs, strengths, and risks for individuals with learning disabilities and/or autism, ensuring a person-centred approach.
- Developing and reviewing personalised care and support plans that promote independence, well-being, and community inclusion.
- Working collaboratively with individuals, their families, carers, and a range of multi-agency partners (e.g., health professionals, housing, education, employment services).
- Safeguarding vulnerable adults and responding effectively to safeguarding concerns, ensuring robust risk management.
- Navigating complex transitions (e.g., from children services, into adulthood, or changes in living arrangements).
- Providing expert advice and guidance on relevant legislation, including the Care Act 2014 and Mental Capacity Act 2005.
- Advocating for the rights and choices of individuals with learning disabilities and autism, empowering them to live fulfilling lives.
