What You’ll Do
- Deliver safe, effective, evidence-based care for registered patients.
- Provide leadership and oversight for practice nurses and HCAs, ensuring compliance with CQC, IPC, and clinical governance standards.
- Manage chronic disease clinics and health promotion initiatives, improving QOF performance.
- Act as an independent prescriber, supporting patient pathways and clinical decision‑making.
- Mentor and develop your team through regular 1:1s, team meetings, and PDR processes.
- Collaborate with practice managers and clinical leads to identify population health needs and implement service improvements.
- Support training and career progression in partnership with our Learning & Experience team.
What We’re Looking For
- Demonstrable advanced practice experience, ideally as an ANP or ACP.
- Experience in primary care or general practice (urgent care experience considered if transferable).
- Proven leadership skills with the ability to manage and motivate teams.
- Excellent communication, adaptability, and resilience – a true people person with a compassionate approach.
- Ability to work flexibly across sites to support service needs.
- Desirable: Teaching/mentorship qualification. Minor illness and chronic disease management courses. Independent Prescriber qualification. Family planning and women’s health qualifications.
