Practice Nurse

Practice nursing is where clinical expertise, continuity of care and prevention come together. The Role

At IC24, we’re looking for an experienced Practice Nurse to join our Primary Care team at Allied Medical Practice and Ship Street Surgery in Brighton. This varied and rewarding role lets you use your full range of nursing skills to deliver high‑quality, patient‑centred care within a supportive multidisciplinary team.

You’ll work across a busy primary care environment, delivering chronic disease management, immunisations, health screening and minor treatments. You’ll also play a key role in prevention, early intervention and health promotion across the local population. Primary care here is not just reactive – it’s connected, designed to improve access, reduce pressure on secondary care and ensure patients receive the right care, in the right place, at the right time.

  • Delivering high‑quality nursing care: Provide assessment, treatment, screening, health education and advice to patients in accordance with the NMC Code of Conduct and practice protocols.
  • Managing long‑term conditions: Diagnose, monitor and support the development of individual management plans for patients with conditions such as diabetes, respiratory disease and cardiovascular disease, in partnership with patients and other professionals.
  • Prevention, screening and health promotion: Undertake NHS Health Checks, cervical screening, immunisations, travel health advice, smoking cessation support and other preventative interventions.
  • Clinical procedures and treatment delivery: Carry out wound care, ear care, phlebotomy, ECGs, blood pressure monitoring and other routine procedures safely and in line with practice policy.
  • Supporting quality and clinical governance: Ensure accurate coding and recording of patient data to support QOF and Enhanced Services and contribute to high standards of clinical governance.
  • Working within a multidisciplinary team: Work independently and in participation with GPs, HCAs, General Practice Assistants and other clinical colleagues to deliver safe, effective and coordinated patient care.
  • Risk assessment and safeguarding: Recognise safeguarding concerns and wider risk issues, including domestic abuse, child protection and vulnerable adults, and ensure appropriate escalation in line with policy.

  • Strong organisational skills and attention to detail in clinical documentation.
