C Support Team.
The Support Worker Team Manager provides high-quality support and positive outcomes for young people leaving care including unaccompanied asylum-seeking young people care, supporting them in their transition into adulthood and independent living.
The Support Worker Team Manager ensures that advice and guidance is provided to Support Workers and young people living independently in their homes.
Auditing and monitoring care plans and working to set Key Performance Objectives (KPO’s), the Support Worker Team Manager is responsible for ensuring that standards, legislation and contractual obligations are met and as set out in the Contract and Law.
- Procedures
- Plan and prioritise your diary to ensure monthly 1-to-1s and performance reviews are completed alongside monitoring of care plans, workforce plan and regular audits.
- Report regularly to the KCC Registered Service Manager and/or the Senior Management Team when required.
Manage own time to meet business demands with a team of support workers located and operating across the Kent area to ensure the productivity of the team.
- Meet own Key Performance Objectives and actively supporting team members to meet their objectives and the requirements of the contract and requirements of Ofsted.
- Review and monitor staff performance against contractual, team and business requirements
- Provide cover for staff absences within the field
- Communicate and liaise with stakeholders and local authority staff and sub-contractors, including the Police, NHS, involved charities and Environmental Health.
- Plan and manage personal business objectives in line with the contract specification and standards set out in the KCC contract
- Ensure the team is trained and developed to support the requirements of the contract and to support the successful completion of their role in line with Ofsted regulations.
- Provide support, advice and guidance to support workers and their young people living independently in their homes.
- Assist in developing young people’s independent living skills.
- Promote and encourage positive life choices in such as social activities, budgeting, health and education.
- Research and maintain an up-to-date library of local services available to assist in the delivery of support to our young people housed in our provision.
- Update and ensure progress is made with care plans accordingly.
- Liaise with social services and other professionals in terms of specific and general service delivery, support, accommodation etc, and to respond to any complaints/issues they have in a timely manner.
- Participate in service developments including planning, control, monitoring, review and evaluation activities to ensure continuous improvements in service delivery.
- Ensure that all young people, and staff are treated in accordance with the Equal Opportunities policy, and fairly with consideration of their cultural, religious and physical needs.
- Work to an agreed rota for on-call assistance as required to young people, staff, social services and management.
- Ensure all safeguarding issues are addressed and signposted in accordance with the company policies and procedures.
- Coordinate and liaise with team members to ensure cohesive and compliant delivery of service.
- Responsible for people management, relating to sickness, absence, capabilities and discipline.
- Provide on call cover during peak times or emergencies.

- NVQ Level 3 in Children’s Social Care and/or equivalent, or willing to work towards one within a set timeframe.
- Knowledge and understanding of The Children’s Act and Children’s Social Care
- A full UK driving licence and use of own vehicle. The role involves working from home with daily travel involved to provide direct support to the K. C. C. Team.
- Must have previous experience and used to working with a young people within the care setting.
- Strong communication skills, in particular the ability to influence, persuade and negotiate at all levels.

- Must be able to travel across a large geographical portfolio on a daily/weekly basis.
- Monitor own work levels, progress and development in the role.
