Are you a Registered Nurse with strong clinical assessment skills who wants to improve outcomes for people with complex and long-term health needs?

We are looking for a CHC Senior Clinical Assessor to join our Continuing Healthcare (CHC) and Complex Care Team. This is an opportunity to use your clinical expertise to support adults with complex needs and help ensure they receive safe, personalised and coordinated care.

You will undertake comprehensive assessments, coordinate and review care packages, and support individuals and their families to understand and navigate the CHC process. Working across health and social care, you will collaborate with multidisciplinary teams, care providers, local authorities and partner organisations to ensure people receive the right support for their assessed needs.

You will also contribute to safeguarding, risk management, quality assurance and service improvement.

We welcome applications from Registered Nurses with strong assessment skills and a sound understanding of the National Framework for NHS Continuing Healthcare who are committed to equitable , person-centred care and improving outcomes for North West London’s diverse communities.

Main duties of the job

As a CHC Senior Clinical Assessor , you will assess, plan, coordinate and review care for adults with complex, long-term health needs in line with the National Framework for NHS Continuing Healthcare and the statutory responsibilities of the ICB.

Your key responsibilities will include:

Undertaking comprehensive clinical assessments to support CHC eligibility decisions.

Developing, coordinating and reviewing personalised packages of care based on assessed needs.

Working collaboratively with individuals, families, multidisciplinary teams, local authorities, care providers and other partners.

Supporting the commissioning and brokerage of appropriate care and monitoring its quality and effectiveness.

Identifying and managing clinical risk and responding appropriately to safeguarding concerns.

Supporting individuals and families to understand CHC decisions, processes and available routes of appeal.

Monitoring care providers and contributing to quality assurance.

Promoting safe, effective and person-centred care for people with complex health and social care needs.

Contributing to service improvement and consistent application of the CHC framework.

Person specification

Education/Qualification

  • Registered Nurse on the NMC register or other health and social care registered professional
  • Educated to degree level
  • Evidence of continuous professional development

Experience

  • Knowledge of National Framework for NHS Continuing healthcare and Nursing Funded Care and associated legislation and guidance
  • Relevant experience supporting people with complex health and care needs, including experience applicable to older people’s services
  • Substantial relevant post-registration experience, with evidence of working autonomously with complex clinical needs
  • Proven clinical Leadership Skills
  • Experience of establishing & maintaining good working relationships
  • Experience of working across multi- agency services
  • Experience of holistic assessment

Skills & Knowledge

  • Ability to communicate, influence and negotiate constructively, including when perspectives differ or change is required.
  • Ability to assess patients with complex needs
  • Ability to lead case conferences/MDT meetings
  • Ability to work autonomously within professional boundaries and seek appropriate support or escalation when required
  • Ability to manage competing priorities
  • Able to manage own workload
  • Able to communicate clearly and effectively both written & verbally
  • Able to deal with challenges and appeals with the support of the Clinical Lead
  • Awareness of Deprivation of Liberty Safeguards -Experience of completing Mental Capacity Act
