Central London Community Healthcare NHS Trust (CLCH) is seeking a Band 7 Community Heart Failure Specialist Nurse to join the Brent and Harrow cardiology service on a 12-month fixed-term, part-time basis at 18.75 hours per week.
You will manage a specialist caseload of patients with complex heart failure, work with GPs and consultants, deliver comprehensive assessments and personalised treatment plans, and contribute to audits, service development and governance.
