Job overview

We have an exciting opportunity to join our established Community Heart Failure service as a Heart Failure Specialist Nurse on a fixed-term contract or secondment until May 2027.

Covering the London Borough of Wandsworth, the service provides complex care for people living with heart failure, working closely with primary and secondary care, community integrated services and other healthcare professionals.

You will use your specialist heart failure knowledge and clinical skills to support patients with complex needs, deliver evidence-based care and provide education that helps people understand and self-manage their condition.

The role involves working across both patients’ homes and clinic-based settings.

Main duties of the job

You will manage a caseload of people with heart failure in the community, using specialist knowledge and clinical skills to assess needs, plan treatment and provide ongoing support and advice.

Working with the wider heart failure team across primary and secondary care, you will deliver evidence-based care and help ensure patients receive coordinated support.

Key duties include:

  • Monitoring patients following discharge to help prevent readmission.
  • Undertaking comprehensive assessments for patients with complex heart failure and co‑morbidities, including physical examination, and developing individualised management and treatment plans using clinical reasoning.
  • Undertaking specialist clinical skills, including administration of medication in line with national and local guidance.
  • Contributing to the development of heart failure care pathways and implementation of relevant NICE guidance.
  • Coordinating education for community health professionals and acting as a specialist resource for advice.

Working for our organisation

We are proud to be one of the largest community healthcare providers in the country, with more than 4,500 colleagues caring for over four million people across London and Hertfordshire. Every day, our teams bring their skill, compassion, and determination to the people who depend on us.

What inspires us is at the heart of who we are: when we work together, we can help people move forward in ways that truly matter. Our teams support children as they take their first steps in life, and they stand beside adults as they rebuild strength, confidence, and independence. From newborn health visiting to community nursing, stroke rehabilitation, and palliative care, we are there for people through some of life’s most important moments.

Joining Central London Community Healthcare means becoming part of a community that lifts each other up. It means working in an organisation that values compassion, welcomes new ideas, and believes in the potential of every colleague. Your development matters here. Your wellbeing matters. Your voice helps shape the future of the care we provide.

We offer a competitive employment package because the work you do matters. At Central London Community Healthcare, you will join an inclusive organisation that invests in its people, supports development, and helps you thrive while delivering high-quality care.
