Job overview

Are you a registered children’s nurse looking to make a meaningful difference for children with complex care and palliative needs?

We are seeking a compassionate and organised Band 6 Kaleidoscope Nurse to help children receive safe, high-quality care in their own homes across Kensington and Chelsea, Hammersmith and Fulham, and Westminster.

Working with the Kaleidoscope Palliative Care Team, Children’s Community Nursing Team and Social Services, you will coordinate home-care packages, support children receiving end-of-life care and help families navigate care with confidence.

You will work autonomously as part of a supportive multidisciplinary service and deputise for the Nurse Specialist to maintain continuity of care.

Applicants must be a Registered Nurse with current UK NMC registration, have relevant nursing experience at Band 5 and evidence of post-registration education. The role is based at Woodfield Road Medical Centre and requires travel, with participation in 24-hour on-call advice and visits for children receiving end-of-life care at home.

Main duties of the job

  • Coordinate the Kaleidoscope service, including allocating Support Workers and arranging bank nurses or care workers when required.
  • Receive and assess requests for home care, determine eligibility and refer families to appropriate services where needed.
  • Complete risk assessments in community settings before care begins and respond to identified risks.
  • Work with community nursing colleagues to arrange, check and maintain essential equipment in the home.
  • Provide 24-hour on-call advice and visits for children receiving end-of-life care at home, alongside the Children’s Community Nursing Team.
  • Work with social workers, commissioners and families to establish and maintain safe packages of care.
  • Support the transition of care packages from children’s to adult services.
  • Develop and review evidence-based protocols, training resources, assessment tools and home-care documentation.
  • Maintain accurate clinical records and service data, including monitoring SystemOne recording.
  • Participate in professional, discharge-planning and service meetings, using interpreters where required.
  • Promote clinical governance, lone-working safety, incident reporting, audit, research and evidence-based practice

Qualifications

  • RGN, RSCN, RN(Child)
  • Registered Nurse with current UK NMC registration
  • Evidence of post registration education
  • Teaching qualification

Experience

  • Relevant nursing experience at Band 5
  • Experience of teaching
  • Experience of liaising / negotiating with professionals
  • Experience of management of junior staff
  • Post Registration Community Experience at Band 5 or 6
  • Experience of working in culturally diverse communities

Skill

  • Knowledge of the NMC Professional Codes and Standards
  • Knowledge of Clinical Governance and its implementation
  • Knowledge of political changes in the profession and NHS
  • Knowledge of working in culturally diverse communities
  • Excellent communication skills, both verbal and written
