We’re recruiting for an experienced Care Coordination Manager to lead and oversee a high‑performing coordination function within a growing complex care provider. This is a pivotal operational leadership role, responsible for workload allocation, rota governance, package mobilisation, and stakeholder engagement across complex and lower‑risk care packages.
This role is ideal for:
- An experienced Care Coordination Manager, Senior Coordinator, or Operations Lead
- Someone with strong workforce planning, scheduling, and people management experience
- A confident leader who can manage competing priorities in a fast‑paced environment
- A professional with strong stakeholder engagement and governance skills
In this role, you will:
- Lead and manage a team of Care Coordinators, allocating workload across complex and lower‑risk packages
- Oversee rota planning, workload balancing, and SLA‑driven task management
- Ensure proactive scheduling, forward planning, and visibility of care delivery
- Act as the main scheduling contact for new package mobilisation and transitions
- Work closely with recruitment, clinical, and operations teams to match carers to packages
- Monitor compliance with care records, data entry, and documentation standards
- Represent the coordination function at MDT meetings and liaise with commissioners and stakeholders
- Track and report key operational metrics including rota gaps, mobilisation timelines, and team performance
- Conduct performance reviews, coaching, and team development
- Identify process improvements and lead service optimisation initiatives
- Participate in the on‑call rota on a rotational basis
What we’re looking for:
- Minimum 4 years’ experience in care coordination, operations, or workforce planning
- Proven people management and leadership experience
- Strong knowledge of rostering, payroll, and care management systems
- Excellent organisational, communication, and stakeholder engagement skills
- Calm, solution‑focused approach under pressure
- Full UK driving licence
Desirable:
- NVQ/QCF Level 3+ in Health & Social Care or Management
- Understanding of CQC regulations and complex care delivery
